Are your child's immunizations up to date?

All students enrolled in Jasper ISD mush have their immunization record on file. Parents will be notified if their child needs to update this information.

Emergency Medical Form

All students must have an emergency medical information sheet on file each year. The school must have the names and phone numbers of persons to contact in the event of an emergency. Parents need to keep this information current.

*Notify the school of change of address or phone number.

Medication at school

Parents need to contact the school nurse on their child's campus to complete the required forms concerning medication to be given at school.

*Medication guidelines are included in student handbooks.
